It’s a win-win situation when you refer family and friends.
We reward you and we also reward them when they come in for their first appointment.
How It Works
- Use the Referral form (above) to submit your information and that of the person you are referring. You can refer as many people you like (but must complete the form per person). A confirmation email will be sent to you on submission of your referral for your records.
- Once we’ve received your referral, we will contact the person(s) to set-up their first appointment.
- After your referred person has completed a new patient exam and hygiene visit, we will contact you regarding your referral success as well as to confirm and arrange delivery (or pick-up) of your gift card(s).
- Referred persons will also receive a welcome gift upon completion of their new patient exam and hygiene. If the person referred does not attend their appointment, we will be unable to issue your reward.
- If you are successful in referring a new patient to our practice, you will receive a $25.00 Amazon gift card.
